A series of up to 10 counseling appointments in an individual setting
This program is based on recommendations from the Academy of Nutrition and Dietetics.
These appointments are for adults and consist of:
An assessment of nutritional status
Education on healthy eating and nutrition habits
Education on weight-management skills for weight loss prior to surgery
Outline of dietary requirements and restrictions that accompany surgery
Counseling on surgery as a tool for weight-management and how to avoid complications and weight regain
Diabetes Education and Self-Management Program
a series of up to 6 counseling appointments in an individual setting
This program is based on recommendations from the American Association of Diabetes Educators (AADE 7).
These appointments are for adults and older children and consist of:
An assessment of nutritional status
Education on healthy eating and nutritional habits
Encouragement for being active
Outline of monitoring blood glucose, according to the most recent guidelines
Tips for taking medications
Help with problem solving for both high and low blood glucose
Healthy coping and keeping a positive attitude
Reducing risks including with the eyes, heart, kidneys, and feet
